A YouTube Description Template That Earns Its Space
Copy this YouTube description template: the three visible lines, chapters, links that get clicked, and the boilerplate that belongs at the bottom.
Here is the YouTube description template, before any explanation, because that is what you came for. It is ordered by what a viewer actually sees: three lines above the fold, then the parts that do work for search and for you.

The three lines of your YouTube description that are visible
YouTube shows roughly the first two or three lines before "Show more". Everything above that fold is read by people deciding whether to keep watching; everything below is read by almost nobody and by YouTube.
So those lines carry the promise, not your Patreon. The most common mistake in this field is a channel that opens every description with the same forty words of boilerplate — spending its only visible space on something a returning viewer already knows and a new one does not care about.

Timestamps in the description become chapters, and chapters get their own presence in search — a viewer can be dropped at the exact minute that answers them. For anything over about four minutes covering more than one point, this is the highest-return block in the template.

A raw link in a description tells you nothing about which video sent the click. Tagged links tell you everything, and cost nothing:

Hashtags in the description render as links above the video title. YouTube shows up to three of them, ignores every hashtag on a video that carries more than 60, and may remove an over-tagged video from search altogether (YouTube Help).

How long should a description be?
Long enough to contain the blocks above; the ceiling is 5,000 characters (YouTube Help) and almost nobody should approach it. A description padded with keyword lists is the 2012 playbook, and it reads as spam to the only audience that matters.

Filling it in without rewriting it every time
The template is only useful if it survives contact with a weekly upload schedule. Two habits make that work.
Write the visible lines last. They restate the promise of the finished video, and you do not reliably know what that is until it is edited. Draft them from the video's own hook rather than from the plan you started with.
Keep the boilerplate in one file, not in your head. The ABOUT block and the standing links change perhaps twice a year. Everything above them changes every video. Treating them as two different things stops you retyping — and stops the boilerplate creeping upward into the visible lines, which is how it usually gets there.

The channel description is a different field
The "About" section on your channel is not the same thing as a video description, has its own fold, and is worth writing once properly rather than pasting from a video.

Do descriptions affect YouTube search ranking?
They give YouTube text to understand the video, and the visible lines affect whether someone keeps watching - which matters more. They are not a place to stuff keywords.
Should the same description be used on every video?
Only the boilerplate block at the bottom. The visible lines and the chapters must be specific to the video, or the most valuable part of the field is wasted.
Where do chapters come from?
The timestamp list in the description, starting at 00:00, in ascending order, with at least three entries. YouTube builds the chapters from that automatically.
